About Us
LifeSpring Care Services
Is an Indiana based organization created in July of 2020. We are dedicated to making a difference in our community by offering person-centered care services to individuals with intellectual and developmental disabilities and other conditions that can limit ability to live a fulfilled life.
What We Do
Residential habilitation Support Services (CIH Waiver)
Residential Habilitation and Support Services provide up to a full day (24-hour basis) of services and/or supports which are designed to ensure the health, safety and welfare of the participant, and assist in the acquisition, improvement, and retention of skills necessary to support participants to live successfully in their own homes.
RHS includes the following activities:
- Direct supervision, monitoring and training to implement the Individualized Support Plan (ISP) outcomes for the participant through the following: Assistance with personal care, meals, shopping, errands, chore and leisure activities and transportation (excluding transportation that is covered under the Medicaid State Plan) Coordination and facilitation of medical and non-medical services to meet healthcare needs, including physician consults, medications, development and oversight of a health plan, utilization of available supports in a cost effective manner and maintenance of each participant’s health record Assurance that direct service staff are aware and active individuals in the development and implementation of ISP and Behavior Support Plans
Structured Family Caregiving (A&D and CIH waivers)
Structured Family Caregiving (SFC) is a caregiving service arrangement in which a waiver participant lives together with a principal caregiver who provides daily care and support to the participant based on the participant’s daily care needs and activities of daily living (ADLs).
Attendant Care (A&D and TBI waivers)
Attendant Care (ATTC) provides direct, hands-on care to participants for non-skilled activities of daily living based on the participant’s specific, assessed care needs which exceeds assistance required for an individual meeting developmental milestone of the same age or life events.
Family Support Waiver (FSW)
The Family Supports home- and community-based services waiver (FSW) provides limited, non-residential support to individuals with developmental disabilities who live with their families or in other settings with informal support.
Transportation
Life Spring Care Services provides transportation for clients so that they may access services they need and in a timely manner
Training / Consultation Services
We offer expert training and consultation services designed to enhance skills and knowledge. Our customized programs and professional guidance empower individuals and organizations to achieve their goals effectively.
